Hospice Team Member

As a Bluegrass Care Navigators' Hospice Team member you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment we also provide care at nursing homes assisted living facilities and hospitals. If a patients care needs are too complicated for home care or require a higher level of attention from nurses and other experts we can provide care at one of our Hospice Care Centers.

Position Summary

The Certified Nurse Assistant (CNA) is an essential member of the hospice care team who works under the supervisor of a Registered Nurse (RN). This role provides personal care and support services to meet the patients daily needs enhance comfort and maintain a safe environment. The CNA is also responsible for observing the patients condition and accurately reporting and documenting observations and the services provided.

Position Qualifications

At least 18 years of age and a high school graduate or equivalent.

Currently registered as a State Nurse Aide with the Kentucky Board of Nursing.

Completion of a recognized Certified Nurse Assistant (CNA) training program.

One year of experience in a hospital or nursing home setting preferred.

Ability to read understand and follow written instructions and accurately document care provided.

Self-directed with the ability to work independently with minimal supervision.

Demonstrates empathy for individuals who are ill frail or impaired and understands the philosophy of hospice care and the needs of the terminally ill.

Must be a licensed driver with an automobile in good working order that is insured in accordance with state minimum requirements.
