Evening Shift Licensed Practical Nurse

About the Role:

As an Evening Shift Licensed Practical Nurse, you'll be responsible for providing direct patient care in our post-acute care setting. This includes administering medications, performing vital signs, and managing treatment plans under the supervision of our experienced nursing staff. You'll also have the opportunity to develop strong relationships with our patients and their families, providing emotional support and education.

Responsibilities:

• Provide high-quality, patient-centered care to our residents in collaboration with our interdisciplinary team

• Administer medications, perform vital signs, and manage treatment plans under the supervision of our experienced nursing staff

• Develop and implement individualized care plans in collaboration with our care team

• Provide emotional support and education to patients and their families

• Participate in quality improvement initiatives to ensure the highest level of care for our patients

• Maintain accurate and detailed records of patient care and treatment plans

Requirements:

• Current LPN licensure in the state of Pennsylvania

• Strong communication and interpersonal skills
