Transition Specialist

Position Overview The Transition Specialist is responsible for providing direct delivery of transition services to students with disabilities. This position has been designated as Safety Sensitive. Pursuant to section 410 of Chapter 4 of the D.C. Personnel Regulations; in addition to the general suitability screening, individuals applying for or occupying safety sensitive positions are subject to the following checks and tests: Criminal background check; Sex Offender Registry check; Pre‑employment drug and alcohol test; Traffic record check (as applicable); Reasonable suspicion drug and alcohol test; and Post‑accident or incident drug and alcohol test. The Transition Specialist will report to the Principal. Essential Duties and Responsibilities The below statements are intended to describe the general nature and scope of work being performed by this position. This is not a complete listing of all responsibilities, duties, and/or skills required. Other duties may be assigned. Provides transition-focused technical assistance and professional development to teachers, IEP team members, parents, students, community agency personnel, and other relevant transition stakeholders. Participates with IEP teams in developing the transition assessment process and the review of IEP goals and objectives. Assists teachers with identifying appropriate transition planning team members, in conjunction with the student and family. Assists teachers in monitoring student, family, and DCPS participation in transition planning and implementation. Evaluates students’ education program with respect to measurable post‑school goals and alignment of those goals with instructional activities. Creates and monitors career exploration activities; arranges tours related to career interests or agency opportunities; and arranges for speakers from adult agencies and organizations to visit schools. Monitors the development and implementation of transition goals and objectives on an ongoing basis. Develops collaborative relationships with external partners and exchanges information to benefit students with IEPs in a seamless transition from school to post secondary opportunities of work, education, and training. Collects data related to the progress of student work‑training experiences and preparation for transition to post secondary options. Develops residential, work‑based, and other community‑based educational programs for individuals with exceptional learning needs. Systematically identifies family service needs related to transition results and assists families in connecting with support networks. Collaborates with and participates in transition-focused interagency coordinating bodies. Qualifications Three to five years of related work experience.
