Job Description:

The RN Case Manager provides comprehensive nursing care coordination and case management services for patients in an inpatient healthcare setting. The role involves assessing patient needs, coordinating care with the interdisciplinary team, facilitating appropriate transitions of care, and supporting quality patient outcomes throughout the hospitalization.

Key Responsibilities:

• Assess patients' clinical, functional, psychosocial, and discharge needs.
• Coordinate individualized plans of care in collaboration with physicians and the interdisciplinary healthcare team.
• Monitor patient progress and identify barriers to treatment, discharge, and continuity of care.
• Collaborate with physicians, nurses, social workers, therapists, and other healthcare professionals.
• Coordinate transitions of care and facilitate safe and appropriate discharge planning.
• Assist with identifying appropriate post-acute services and community resources.
• Monitor utilization of healthcare services and support appropriate resource management.
• Communicate patient status, care plans, and discharge needs with appropriate members of the care team.
• Educate patients and families regarding care plans, treatment needs, follow-up, and available resources.
• Document assessments, interventions, care coordination activities, and discharge planning accurately.
• Advocate for patients and help address barriers that may affect access to appropriate care.
• Maintain compliance with applicable nursing, case management, and organizational standards.

Required Qualifications:

• Active, unrestricted California RN License .
• BLS certification .
• Minimum 1 year of inpatient nursing experience .
• Experience with patient assessment, care coordination, and interdisciplinary collaboration.
