With deep roots in Western Massachusetts and a growing presence in communities throughout the state, we are currently seeking a Licensed Practical Nurse (LPN) to join our Program for Assertive Community Treatment (PACT) team and help advance our mission of empowering people living with serious mental illness to live healthier, more independent lives in the community.What We Offer:Meaningful Impact – Provide direct, hands-on nursing care that supports the health, recovery, and community integration of individuals with serious and persistent mental illnessCollaborative Clinical Team – Work under the guidance of the Medical Director and PACT Team Leader alongside psychiatrists, nurses, case managers, and peer specialists in a true team-based modelCommunity-Based Practice – Deliver care where individuals live, work, and spend their time, rather than solely in a clinical office settingSupportive Team Culture – Join a team guided by our HEART values: Humanity, Empowerment, Accountability, Respect, and TeamworkComprehensive Benefits – Generous PTO, 403(b), educational assistance, medical, dental, vision, employee assistance programs, and moreCertified Great Place to Work – Work for an organization recognized for creating a positive and engaging employee experienceWhat You Will Do:Participate in data collection related to the psychiatric, medical, and functional status and needs of individuals served on the PACT teamProvide direct nursing care within scope of practice, including administering and monitoring psychiatric and medical medications, both in-office and in community settings (homes, shelters, and other locations)Educate individuals served and their families on mental illness, co-occurring medical conditions, treatment, and prognosis, reinforcing teaching under the direction of a Registered Nurse or Nurse PractitionerSupport medication adherence strategies, including injectable medication administration, blister-packing, and home delivery of medications as neededThoroughly and accurately document all aspects of care in the individual's record, communicating pertinent information to the multidisciplinary team at daily organizational meetings and shift reportMaintain strict confidentiality of individuals served information in accordance with 42 CFRProvide health assessments and monitor chronic medical conditions for individuals served, coordinating with primary care and specialty providers as neededPrepare and submit nursing assessments and progress notes in accordance with organizational and regulatory policyMonitor medication logs for accuracy and use EMR/staff logs to convey pertinent clinical informationServe as liaison to area hospitals, emergency departments, and medical facilities, and act as a resource for individuals served and their families regarding physical and behavioral health needsParticipate in crisis intervention and outreach as part of the PACT team's 24/7 availability, including community-based visits as scheduledProvide ongoing in-service training and consultation to staff, and participate in continuing education and staff development activitiesWho You Are:High School Diploma or GED requiredCurrent Massachusetts Board of Registration license as a Licensed Practical NurseMinimum one year of nursing experience preferred; experience with psychiatric or behavioral health populations a plusSkilled in applying the nursing process to individuals served and their families, including those with serious mental illness and co-occurring substance use or medical conditionsComfortable providing services in community settings, including individuals' homes and other non-clinical environmentsAble to collaborate with and appropriately delegate to other caregivers within a multidisciplinary teamKnowledgeable in utilization/review standards, quality assurance, and applicable regulatory standardsAble to understand, empathize, and work effectively with individuals experiencing serious mental illness, including those with co-occurring disordersWorking knowledge of the ethics and legal parameters of mental health and substance use treatmentValid driver's license requiredExcellent verbal and written communication skillsWe Hire for HEART!
