Field-Based Behavioral Health Coordinator - Foster Care

Elevance Health

Elevance Health seeks a field-based Managed Care Coordinator (BSW, MSW, or QMHP) to manage members' individual service plans within state law and contract scope. The role involves telephonic or face-to-face assessments, coordinating care with clinical teams, and addressing non-clinical needs for members with chronic illnesses.
