Licensed Practical Nurse (LPN) - Itasca, IL

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Landmark of Itasca, a leading provider of long-term care services in Itasca, IL. As an LPN, you will play a critical role in providing direct patient care to our residents, supervising nursing assistants, and ensuring the highest level of quality care is maintained at all times.

Responsibilities:

• Direct patient care to ensure the health, safety, and well-being of our residents

• Supervise and mentor nursing assistants, providing guidance and support as needed

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Perform various nursing procedures, including administering medications, wound care, and basic life support

• Maintain accurate and detailed records of patient care and communicate with healthcare providers as necessary

• Participate in Continuous Quality Improvement (CQI) initiatives to improve patient outcomes and reduce readmissions

• Collaborate with other healthcare professionals and team members to deliver comprehensive care

• Ensure compliance with facility policies, procedures, and regulatory requirements

Requirements:

• Must possess a current, unencumbered LPN license in the state of Illinois

• Minimum of 1 year of experience as an LPN, preferred

• Possess strong communication and interpersonal skills

• Ability to work in a fast-paced environment with frequent interruptions

• Commitment to providing high-quality patient care and adhering to facility policies and procedures

• Ability to work varied shifts, including evenings, weekends, and holidays

Work Environment:

• Work in an office setting and throughout the nursing service area

• May be subject to frequent interruptions and hostile or emotional situations

• May be required to work beyond normal working hours, including evenings, weekends, and holidays

• May be exposed to infectious waste, diseases, and conditions, including TB, AIDS, and Hepatitis B Virus

Physical Requirements:

• Ability to move intermittently throughout the work day

• Ability to cope with mental and emotional stress of the position

• Ability to see and hear with or without aid

• Ability to push, pull, move, and/or lift a minimum of thirty-five pounds
