Hospice Medical Social Work Liaison - Care Coordinator

Ohio's Hospice, Inc. is seeking a Social Work Liaison to work with discharge planners, physicians, palliative care team members, social workers and RN unit staff to assist with admitting patients to the Ohio’s Hospice program and to facilitate safe transfers to ECF, Home or Hospice Care Center. Based primarily at the inpatient unit at Ohio State University Wexner Medical Center with travel to our inpatient care center at Licking Memorial Hospital, the role involves psychosocial assessment, care #J-18808-Ljbffr Ohio's Hospice, Inc.
