Place of Performance- FCI Sheridan 27072 SW Ballston Rd. Sheridan, OR 97378

Schedule- Pharmacist-These services will be required five (5) days per week, normally consisting of approximately eight, one (1) hour sessions per day, Monday through Friday (except Federal holidays), normally between the hours of 6:00am-8:00pm.

The Contracting Officer’s Representative (COR) will be responsible for defining each weekly schedule. The schedule is subject to change based on the variable needs of the institution and the unpredictable nature of the correctional environment, and a single contractor may be asked to work up to, but not exceeding, forty (40) sessions per week (80 sessions per two weeks). Upon agreement, any changes to the schedule must be coordinated with the COR assigned. A mutually agreed upon schedule will be determined following award. The Contractor may be subject to schedule changes and must be flexible.

Education and Experience- Pharmacist-The Pharmacist must possess an unencumbered license to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Qualified applicants must provide verification of credentials, including, but not limited to, resume, education, current license and/or certification, competency, continuing education, and proof of insurance as required by the Accreditation Association for Ambulatory Health Care (AAAHC) and the Bureau of Prisons Program Statements. The contractor must be Basic Life Support Certified in CPR, in accordance with the American Red Cross or the American Heart Association. The contractor must provide two professional written references from practice in the profession within the last five years. The contractor must submit all credentialing information as soon as possible, since there are additional BOP verifications that must be satisfied, including the National Practitioner Data Bank and Primary Source Verification (PSV).

Duties-All duties shall be performed in accordance with standards and methods generally accepted within the field of Pharmacist services, and compliance with all Federal and State policy and regulations.

The Contractor shall adhere to all standards and regulations associated with Accreditation Association for Ambulatory Health Care (AAAHC) and American Correctional Association Foundation/Core Standards for Adult Correctional Institutions. The manner and method of performance are the sole responsibility and within the sole discretion of the Contractor, provided, the Contractor performs the services in accordance with standards and practices consistent with those in the community. The Contractor’s performance shall be monitored by the COR.

The services provided will be provided in the method commensurate with the local community. These services, although not directly supervised, will be reviewed by a pharmacist on staff at FCI Sheridan. The performance of these services will also be verified by the designated COR.
