Seven Healthcare are seeking a dedicated and skilled Physical Therapist for a contract assignment in Owensboro, KY. Specializing in home care, this role involves performing physical therapy evaluations and treatments based on physician orders. The successful candidate will work primarily in a home-based environment, ensuring optimal patient outcomes through comprehensive assessments and targeted treatment plans.

Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluations and treatments of patients based on physician's referrals.
• Complete comprehensive evaluations using appropriate assessment tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans to enhance patient independence and functionality.
• Regularly reassess treatment plans and patient progress, modifying goals or treatments as needed.
• Communicate patient needs and progress to the treatment team, physician, and family members.
• Consult with medical providers regarding patient evaluation, treatment, progress, and discharge.
• Recommend and assist in securing special adaptive equipment to improve patient function.
• Maintain and submit required evaluation, treatment, and billing documentation.
• Adhere to the Company's Code of Business Conduct and all applicable state and federal laws.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Participate in staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist

• KY PT License or Compact License accepted
• Experience with Home Care/Home Health
• Medicare Oasis experience preferred
• PT skills
