Capital Hospice & Palliative Care - Bereavement Coordinator

Job Title/Position: Bereavement Coordinator Reports To: Clinical Director Job Description Summary The Bereavement Coordinator is responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families and caregivers from Capital Hospice and Palliative Care LLC for up to one year following the death of the hospice patient. Essential Job Functions / Responsibilities Coordinate the assessment and delivery of grief counseling services (one‑to‑one, groups, and ongoing follow‑up) to ensure timely and appropriate support for families and caregivers. Plan bereavement program development, expansion, and refinement through annual evaluation of services. Provide bereavement support to hospice personnel coping with work‑related grief through one‑to‑one counseling, supervise interns providing counseling, and refer to community resources. Provide bereavement information and referral services to callers from the community; expand and maintain community resource list as needed. Assist in evaluating and planning services for hospice personnel support (e.g., yearly retreats, weekly support groups). Work independently while seeking and accepting guidance from interdisciplinary team members or community members. Participate in hospice activities such as in‑service education, personnel meetings, and relevant committees. Advance professional knowledge and skills. Actively participate in quality assessment and performance improvement teams and activities. Accept and perform other related duties and responsibilities as required. Position Qualifications Bachelor's degree or equivalent theological degree plus experience working with families and patients through death and dying issues, OR a Master’s degree in social work with a specialization in clinical grief counseling. Ability to work as a team member. Knowledge and commitment to hospice philosophy. Demonstrated verbal and written communication skills and organizational abilities. Understanding of grief response and experience counseling individuals and/or groups regarding loss. If making home visits, must be a licensed driver with an insured automobile that meets state or organization requirements and is in good working order. #J-18808-Ljbffr
