Clinical Coordinator
The Clinical Coordinator provides critical administrative and scheduling support to ensure timely, compliant, and high-quality nursing care across the branch. This role is responsible for scheduling all nursing visits, including routine care, reassessments, and initial assessments, while maintaining clear communication with nurses, clients, and caregivers to ensure visit completion and documentation timeliness.
Responsibilities:
- Manage the preparation and maintenance of clinical documentation, which includes printing and assembling admission packets
- Support nurses in ensuring documentation accuracy and compliance.
- Oversee the upload, filing, and tracking of all care delivery paperwork.
- Maintain and update all client files in accordance with 245-D licensing rules and regulations.
- Review documentation to confirm all required signatures are present, ensuring full compliance with 245-D licensing rules and regulations.
- Manage the 30-60-90-day reassessment tracking system, guaranteeing all required visits are scheduled and completed within regulatory timelines.
- Collaborate with the Regional Clinical Manager and the branch nursing team to streamline clinical operations and ensure documentation is accurate, compliant, and well-organized.
- Prepare quarterly and annual reports as requested by the person served and/or the team to support the person-centered plan, when needed.
- Demonstrate strong, creative problem-solving skills.
- Perform all necessary physical demands of the position (e.g., lifting, transfers).
- Complete other assigned duties as needed.

Qualifications:
- High school diploma or equivalent required
- Familiarity with EMR systems
Job Type:
- The typical work schedule for this role is Monday to Friday, from 8:00 a.m. to 4:30 p.m.
- Additional hours may be required outside of this standard schedule, based on the operational needs of the office.
