Work Environment

This position involves onsite work, focusing on home care, where you will deliver therapy services in patients' residences. Key Responsibilities

• Assess and evaluate patients' conditions, establishing treatment plans aimed at preventing disability, alleviating pain, and restoring physical function.
• Direct and supervise Physical Therapy Assistants to ensure adherence to tailored therapy plans.
• Conduct diagnostic evaluations to understand a patient’s muscle, nerve, and joint capabilities, adjusting therapeutic approaches as necessary.
• Provide physical therapy treatments, including manual and non-manual exercises, traction, massage, and other physical agents, all while keeping the patient's comfort and safety a top priority.
• Foster patient education by instructing clients and their families on therapy methods and the use of assistive devices, enhancing their understanding and engagement in the rehabilitation process.
• Document patient progress meticulously, preparing clinical records such as evaluations, care plans, and discharge summaries for ongoing assessments and interventions.
• Participate in interdisciplinary team meetings to design and review comprehensive patient care plans, making recommendations based on evaluations and observed progress.
• Ensure compliance with state and federal regulations while maintaining the integrity of patient information and care standards.
• Adapt communication strategies to effectively address the needs of diverse patient populations.
• Complete administrative tasks including timely submission of clinical documentation and adhering to branch policies.

Patient Population / Client Focus

As a Physical Therapist at Motion Menders Therapy, you will work primarily with diverse patients requiring rehabilitative care in their homes. This includes individuals recovering from surgeries, those with chronic conditions, and elderly patients needing assistance to improve or maintain their mobility and independence. Your expertise will be essential in tailoring therapy programs that best serve each patient's specific circumstances.

Qualifications

• Graduation from an accredited physical therapy program.
• Current licensure to practice as a Physical Therapist in Florida, maintaining good standing.
• Minimum of 2 years of relevant experience, ideally in a home care setting, reflecting a solid understanding of unique patient needs.
• Valid CPR certification, current driver’s license, and proof of automobile insurance are mandatory.
• Strong interpersonal and communication skills to connect effectively with patients and families.
• Ability to engage in critical thinking and problem-solving, adapting methodologies to meet individual patient needs.
• A passion for providing high-quality patient care and commitment to continuous personal and professional growth.

Preferred Experience

• Experience working in home care or acute care settings, with a focus on geriatric and diverse populations.
• Previous involvement in interdisciplinary team settings to foster collaborative care approaches.
• Familiarity with documentation systems for patient evaluations and care plans, particularly OASIS assessments.

Compensation and Benefits
