Become a member of our dedicated primary care team at a Federally Qualified Community Health Center situated in Grand Junction, Colorado, on the beautiful Western Slope! We are in search of a Board Certified or Board Eligible Family Medicine Physician who is passionate about delivering integrated, quality healthcare to everyone. Our facilities offer a comprehensive array of primary and preventive healthcare services, including medical, dental, behavioral health, psychiatric, and pharmacy care. Currently, we operate two integrated primary care clinics and two school-based health centers in Grand Junction. Our organization has a mission-driven history of providing outreach healthcare services to individuals experiencing homelessness and migrant farm workers. We accept patients with Medicaid, Medicare, Medicare Advantage, commercial insurance, as well as those who are uninsured or underinsured. A Sliding Fee Discount Scale is available for qualifying patients.

Position Highlights:

• This is a full-time, in-person role.

• Enjoy a 4-day workweek with an average patient load of 15-17 per day (4x10 schedule), with alternative scheduling options available.

• Work weekdays only—no weekends or on-call duties. Focus exclusively on outpatient care.

• Collaborate with Behavioral Health providers within each clinic pod.

• Be part of a strong, team-oriented clinic staff that includes Medical Assistants, Registered Nurses, patient navigators, case managers, and care coordinators.

• Access on-site pharmacy, dental, and psychiatric services.

• Participate in an active Quality program with impressive outcomes.

• Engage in a broad scope of practice, including wellness visits, chronic disease management, and opportunities for in-office procedures based on your interests.

• Benefit from a collaborative environment with weekly Lunch & Learns.

Candidate Qualifications:

• Must be Board Certified or Board Eligible in Family Medicine (ABMS).

• Possess an unrestricted Colorado physician license and DEA license.

• Enjoy providing care to patients of all ages.

• Experience in integrated team-based care is preferred, with prior FQHC experience being a plus.

• Bilingual skills are advantageous.

Compensation & Benefits:

The salary range for this position is between $196,896 and $266,875.
