Overview Nurse Practitioner (NP), Hospice – Hartford County, MD. Coverage Area: Bel Air, Belcamp and surrounding areas. Remote/Virtual Position: No. Full‑time Hospice Nurse Practitioner. What You Need to Know Be the Best Hospice Nurse Practitioner You Can Be If you meet these qualifications, we would love to meet you: Graduated from an approved school of professional nursing – master’s degree Current and unencumbered advanced practice nurse license in the state(s) practicing One year of experience as an NP Certified by a nationally recognized advanced nurse practice organization Possess and maintain valid CPR certification while employed in a clinical role (preferred) Must be a licensed driver who can travel to all business locations Meet the regulations and requirements of the state(s) in which the program provides services Responsibilities As a Hospice Nurse Practitioner, you will: Initiate communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family. Monitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teaching. Maintain continuity of care in coordinating with the care team and case manager for patients with ongoing needs. Prescribe medication as permitted by the state and/or the state Board of Nursing. Pre‑Hospice Face‑to‑face clinical and needs assessment at intake, preferably in the patient’s home setting, inclusive of medical, emotional, spiritual, social, and practical needs. Face‑to‑face reassessment done when needed and at least every 60 days; plan of care development based on these assessments and incorporating the patient’s and family’s priorities. Initiate communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family. Clinical supervision of the care plan including all medicine orders. Collaborate with the rest of the pre‑hospice team, alerting the team to changes and newly identified patient needs as they arise.
