RN Case Manager

Home Health/Hospice RNThe home health/hospice RN will case manage and provide skilled nursing services for patient visits. This will include coordination of home care services to patients, supervision of CNAs and PRN nurses in the home, and coordination of care between home care disciplines (PT, OT, ST, CNA, MSW).
