- Performing thorough patient evaluations, ordering necessary diagnostics, and developing customized treatment strategies
- Diagnosing and addressing acute, episodic, or chronic conditions jointly with physicians or independently (as verified by credentialing)
- Providing education to patients and families regarding prevention, wellness, and chronic disease management
- Administering prescriptions and therapies within your practice's guidelines
- Engaging in patient follow-ups and chronic condition management
- Working collaboratively within interdisciplinary teams to provide seamless, comprehensive care
- Documenting diligently and accurately in electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Completion of an accredited Nurse Practitioner program
- Valid Nurse Practitioner licensure in the state of Illinois
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a suitable equivalent
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, or the ability to obtain these within 90 days
- A solid clinical background complemented by a caring demeanor
- Exceptional communication abilities with patients, families, and healthcare teams
- Flexibility in fast-paced, patient-centered environments
- Familiarity with EMR systems and telehealth technology
