Physical Therapist (PT)

Job Summary

We are seeking a dedicated Physical Therapist to provide high-quality home health care to patients. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work in various home environments.

Job Details

• Unit: Home Health
• Shift: Monday to Friday, 8:30 AM to 5:00 PM
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours per week
• Required Experience: Minimum of 2 years in a healthcare setting, with home health experience preferred

Responsibilities

• Deliver physical therapy treatments in patients' homes, assisted living facilities, senior apartments, and board and care homes.
• Manage a caseload of 3 to 5 patients per day, including 2 Start of Care visits and 3 to 4 evaluations.
• Complete all required documentation and case management within the shift hours.
• Work in collaboration with a RN Manager and support from a Regional Rehab Leader/Educator.
• Supervise up to 2 Physical Therapy Assistants (PTAs) or Certified Occupational Therapy Assistants (COTAs).

Qualifications

• Current California Physical Therapist license is required.
• Basic Life Support (BLS) certification from the American Heart Association is mandatory.
• Experience with HomeCare Home Base (HCHB) EMR is preferred.
• Knowledge of OASIS assessments is required.
• Must possess a valid driver's license and proof of automobile insurance.
• Ability to work effectively in diverse home environments.

Additional Information

• Orientation will include 3 weeks of training with both online learning and field precepting.
• A weekend shift is required once per month, though this is flexible.
• Holiday availability is expected, with some flexibility for time off requests.
• Professional attire is required; scrubs are recommended. Closed-toe shoes are mandatory.
