CAP Case Manager

CAP Case Manager AuthoraCare Collective is currently seeking a CAP Case Manager. This is a full‑time hybrid position, Monday – Friday 8:00 am – 5:00 pm. Manage the overall plan of care for a minimum case load assignment of CAP‑C and/or CAP‑DA recipients as determined by the Director of Kids Path. Assess and identify recipient/caregiver problems, establish goals, and implement interventions. Conduct monthly recipient visits for assessment of ongoing needs, interventions, and service utilization. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Responsibilities Direct and coordinate all services provided through CAP‑C and CAP‑DA to recipients in accordance with the individualized care plan. Utilize the case‑management process in planning and prioritizing recipient care needs, including assessment and identification of recipient/caregiver problems; identification and implementation of interventions; and evaluation of outcomes. Partner with the assigned RN case manager to complete the annual recertification process. Establish and maintain relationships with recipients/caregivers to adequately assess ongoing needs. Recognize the influence of culture, ethics, and beliefs on decision making. Make home visits in accordance with NC Medicaid requirements. Offer telephonic support to recipients and caregivers as needed. Communicate changes to the plan of care to recipients/families and stakeholders. Comply with infection control and exposure control policies. Maintain effective relationships and partnerships with community healthcare providers and external stakeholders impacting the CAP‑C and CAP‑DA population. Qualifications Bachelor's degree in social work or a similar healthcare/psychosocial field. Two years of experience in social work case management. Two additional years of pediatric case management experience (preferred). Case‑management accreditation or certification (preferred). Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by agency and have own transportation.
