What You'll Do

• Evaluate residents and develop individualized treatment plans
• Help residents improve mobility, strength, balance, and independence
• Collaborate with nursing and care teams to support resident outcomes
• Educate residents and families on treatment goals and progress
• Maintain accurate OASIS documentation and therapy records

What We're Looking For

• Active Pennsylvania physical therapist license required
• Experience in home health or short-term rehabilitation is preferred but not required
• Strong communication and problem-solving skills
• Ability to work independently and as part of a team
• Passion for helping seniors achieve their highest level of function
