The provider will manage a diverse endocrinology patient population, primarily focused on diabetes management, osteoporosis, and transgender care. This is an excellent opportunity for a qualified provider to join an established healthcare team and deliver both in-person and telehealth services. Key Responsibilities Evaluate, diagnose, and manage patients with Type 1 and Type 2 diabetes, including acute diabetic conditions. Provide care for patients with osteoporosis and other endocrinology-related conditions. Support transgender patients requiring endocrinology care. Conduct new patient consultations and follow-up appointments. Deliver care through both in-person visits and telehealth consultations. Maintain accurate patient records using EPIC (Health Connect). Collaborate with the healthcare team to develop and implement individualized treatment plans. Requirements & Qualifications Active California NP or PA license required. Board certification required. Current American Heart Association (AHA) BLS certification. Experience in endocrinology and diabetes management preferred. Experience treating both Type 1 and Type 2 diabetes strongly preferred. Diabetic Educator Certification is a plus. Ability to provide both in-person and telehealth care.
