About the Role:
As a Social Services Assistant Designee, you will work closely with our compassionate team to coordinate social services, support, and resources for our residents. Your primary responsibilities will include:
• Assessing and addressing the social, emotional, and mental health needs of our residents
• Developing and implementing personalized care plans to promote wellness and independence
• Collaborating with our interdisciplinary team to ensure seamless care transitions and communication
• Identifying and addressing potential social and environmental barriers to patient care
