Physical Therapist

8-hour shifts.

Assignment Details

Contract length: 13 weeks. Guaranteed hours: 40 per week.

Requirements

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license with reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Responsibilities

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document and complete a Plan of Care (POC), progress notes, and other clinical record documentation.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on physician referrals.
• Assist and instruct patients and families in transfer techniques and the use of therapeutic equipment.
• Provide patient and family education regarding therapeutic procedures.
• Oversee other team members in delivering patient care consistent with patient needs.
