A community service organization in New York is seeking a Case Manager to provide direct case management services to homeless individuals. The role includes conducting intakes, developing treatment plans, and providing advocacy. Candidates should have a bachelor's degree in a related field and three to five years of experience in case management, preferably with vulnerable populations.
