Pathways Home Clinician: Community Transition Specialist
POSTGRADUATE CENTER FOR MENTAL HEAL
POSTGRADUATE CENTER FOR MENTAL HEAL is seeking a Licensed Mental Health Professional to work with individuals transitioning from Adult Homes into the community. The role involves assessments and the development of treatment plans while providing group and individual counseling. You will foster relationships with community providers to ensure linked services, provide crisis intervention, and assist with housing placements.
