Physical Therapist

About the Role Physical Therapist for the BoldAge PACE program, an all-inclusive care initiative that supports seniors in maintaining independence at home. Responsibilities Plans, organizes, and implements physical therapy services for PACE participants. Performs in‑person assessments at initial enrollment and as needed. Assesses the need for adaptive durable medical equipment, specialized shoes, and prostheses. Evaluates the participant’s home for safety and mobility, recommending modifications and equipment. Plans physical therapy treatment programs according to recognized standards and institutional policies. Administers treatment programs, consistently achieving identified goals. Collaborates with the interdisciplinary team (IDT) on participant care issues. Documents evaluation, assessment, plan, and treatment accurately and timely per policy. Monitors treatment effectiveness, documents results, and revises the plan accordingly. Educates participants, families, and caregivers on procedures, home programs, and ensures understanding. Trains PACE staff in body mechanics, equipment use, transfers, gait patterns, and methods to maximize safety and functional abilities in activities of daily living (ADL's). Provides care in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care. Assists participants to function at the most independent level possible. Develops and maintains positive relationships and communication with co‑workers, participants, families, and community members. Participates in daily IDT meetings, communicating participant changes and collaborating on care planning decisions and 24‑hour care coordination. Performs duties of other IDT members as needed based on licensure, competencies, and experience. Engages in quality improvement projects and meetings. Attends organizational meetings related to participant care, daily operations, in‑services, and community agency meetings. Completes all documentation of clinical service in participants’ medical records, including initial assessments, reassessments, changes of status, and significant events per PACE documentation requirements. Follows all policies and procedures, OSHA safety guidelines, protects privacy, and maintains confidentiality of patient information. Participates in continuing education, maintains professional affiliations, certifications, and ongoing training requirements. Performs other duties as assigned. Qualifications Advanced degree in Physical Therapy from an accredited school. Current state license to practice Physical Therapy. Current driver’s license and proof of auto insurance. Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community setting preferred. One (1) year of experience working with a frail or elderly population preferred; training will be provided if not present.
