Position: Physical Therapist Assistant (PTA)

We are seeking a dedicated and compassionate Physical Therapist Assistant for a home health position that involves providing therapy services to patients in their homes. The selected candidate will participate in various patient care activities and documentation tasks, while maintaining a clear communication with the interdisciplinary team.

Key Responsibilities:

• Conduct home visits to provide physical therapy assistance to patients.
• Document patient progress and updates according to established protocols.
• Participate in periodic office meetings.
• Work collaboratively with other healthcare professionals to develop and implement care plans.

Schedule:

• Work hours: Monday to Friday, 8:00 AM - 5:00 PM
• Type of schedule: Day 5x8-Hour
• Travel required: Up to 75 miles from home clinic.

Patient Information:

• Average patient visits per day: 5-6
• Types of cases include Start of Care and Follow Ups.
• Daily productivity expectations are 30 points per week, equating to a minimum of 36 patient visits.

Qualifications:

• Minimum of 1 full year of adult home health experience is required. First-timers are not accepted.
• Certifications: Current CPR and BLS certifications are mandatory.
• Must have reliable transportation, a valid driver's license, and agency-required liability insurance.
• Experience with OASIS and HomeCare HomeBase EMR is required, with proficiency within the last 6 months.

On Call Requirements:

No on-call duty is required for this position.

Work Environment:

This position primarily involves working in patients' homes, ensuring a comfortable and familiar setting for rehabilitation.

Location:

Griffin, GA with potential to float to surrounding counties including Spalding, Lamar, Upson, Meriwether, and Pike.
