Duties & Responsiblities:

• Serve as a liaison between CREOKS Health Services and inpatient/crisis/PRTF/substance use recovery programs to provide transitional support for adults and children discharging to CREOKS, to include identifying those qualifying for the suicide prevention care pathway.
• Establish and maintain collaborative relationships with the above institutions to develop referral networks.
• Ensure persons discharged are connected with the indicated programs and clinics for follow up.
• As part of contacts during transition, the liaison will: review the discharge summary and instructions; perform medication reconciliation; ensure that follow-up appointments are scheduled and coordinated; and assess the person's risk for readmission to the above-mentioned facilities or risks of failure to attain community-based care/tenure.
• Ensure that those persons on the suicide prevention care pathway discharging to CREOKS have a follow up contact by either phone, zoom, face-to-face, or caring card within 12-24 hours post discharge.
• Communicate appropriately with local site directors and intake staff to maintain continuity of care.
• Assist the Centralized Scheduling Coordinator in creating Hospital Discharge Tracking form for all hospital discharge referrals.
• Assist the Centralized Scheduling Coordinator with sending out weekly hospital discharge emails.
• Maintain updated clinic points of contact.
• Represent the Centralized Scheduling/Intake department to local CREOKS sites and community partners/referring entities.
• Use verbal de-escalation skills, functional assessments, crisis planning/safety planning and/or community resources to assist persons in managing and/or gaining control of a crisis either via telephone/within the clinic within the home or community.
• Monitor discharge paperwork retrieval from referring programs and ensure that hospital discharge paperwork and medication lists are received and uploaded to client charts.
• Provide advocacy, linkage, and referral services on behalf of chronically mentally ill.
• Provide intake, screening, referral and PRSS services.
• Will assume a whole person approach and will screen for medical care needs, link to medical providers, and provide health promotion education consistent with their level of training and experience.
• Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations.
• To read and be knowledgeable of all current CREOKS policies and procedures.
• Participate in Transition Services and Zero Suicide monthly meetings.
• Turn in billing and associated documentation as mandated by CREOKS policy.
• Responsible for understanding and following all of the administrative, clinical, and programmatic policies and procedures of the agency.
• Responsible for necessary interaction with agency program participant families and/or other agencies involved with participants to ensure appropriate therapeutic care.
• Provide services and supports to all program participants within the community (to include a participant’s residence, etc.) consistent with the Recovery Support Specialist’s education, experience, training, and scope of practice.
• All other duties as assigned.

Qualifications/Competencies Required:

• Must be in recovery for at least one (1) year from mental illness and/or co-occurring disorders.
• High school diploma or GED.
• Ability to conduct outreach to different providers and promote CREOKS services.
• Ability to provide presentations to the other providers and the community about CREOKS services.
• Ability to communicate needs to others.
• Ability to share story of recovery to individuals and groups.
• Knowledgeable of mental illnesses and co-occurring disorders.
• Successfully complete the ODMHSAS Came Management and credentialed as a CM1
• Become trained and certified as a Wellness Coach according to ODMHSAS standards.
• Successfully complete the Tobacco Free Support Group Training.
