LPN - Population Health

Role Overview We are hiring an LPN for our Population Health Division to serve as an Outreach Specialist. You will coordinate care for low to moderate risk patients after hospital discharge, conducting follow‑up calls and optimizing health management. Responsibilities Schedule and conduct 8‑hour day shifts, Monday through Friday. Work hybrid: 3 days a week after 90 days of independent work. No nights or weekends. Locality: Primary Care Clinic in Independence, MO or Platte City, MO. Monitor patient progress, order follow‑up appointments, and provide education. Maintain accurate documentation and reports. Qualifications Licensed Practical Nurse (MO) with a valid nursing license. 3–5 years of nursing experience. Strong communication skills and ability to work independently.
