Clinical responsibilities include conducting initial psychiatric evaluations of 60 minutes and medication management visits of 30 minutes for adults with serious mental illness (SMI) who are transitioning out of acute psychiatric hospitalization. The primary clinical goal is to stabilize patients and support prevention of re-hospitalization within a community-based, step-down care framework. The EMR system utilized for this assignment is AVATAR. No call requirements are associated with this position.
