Role Description The Social Worker is a full-time, on-site role based in Myrtle Beach, SC, providing direct clinical and supportive services to patients. Day-to-day responsibilities include conducting psychosocial assessments, developing treatment and care plans, and delivering individual and family counseling. The Social Worker will coordinate with physicians, therapists, and other care team members to ensure integrated, holistic care. Additional duties involve maintaining accurate clinical documentation, advocating for patient needs, connecting individuals with community resources, and participating in case conferences and team meetings. The role may also include crisis intervention, safety planning, and contributing to quality improvement initiatives within the practice. Qualifications Demonstrated ability in patient assessment, care planning, counseling, and case management. Strong skills in communication, collaboration, and relationship-building with patients, families, and interdisciplinary teams. Proficiency in clinical documentation, time management, and organization in a fast-paced healthcare environment. Knowledge of mental health conditions, therapeutic approaches, and community resources relevant to behavioral health. Master’s degree in Social Work (MSW) from an accredited program; active, unrestricted social work license in South Carolina or license eligibility. Experience in medical, mental health, or behavioral health settings is highly beneficial. Commitment to ethical practice, cultural humility, and trauma-informed, person-centered care.
