Responsibilities Serves as a member of the management team for planning and implementing program development, program goals and participant outcomes Supervises Residential Care Coordinators and performs all duties inherent in supervisory roleEnsures medical records, treatment planning and documentation are consistent at all assigned residential programs in the continuumPerforms all duties of Care Coordination, as needed to includeFormulates treatment plans, functional outcomes/goals and service delivery for referred clients in conjunction with treatment teamParticipates in treatment team meetings to ensure coordination of careManages the provision of individual client services, ensuring client’s needs are metIntervenes and problem solves issues if outcomes/goals and service delivery are not being met according to treatment plan and established timelinesDevelops client’s initial, progress and discharge reports; tracking and documenting status of person servedSchedules/facilitates interdisciplinary case conferences with all therapists, payers and family membersCommunicates regularly with family, payer and physician regarding client progressMonitors client, family, physician and payer satisfaction with the program on a regular basisArranges for provision of necessary equipment to enable achievement of agreed upon functional goals (i.e. Health Maintenance Organization, insurance carrier) with funding sourceParticipates in Program Evaluation, Quality Assurance, and in establishing the treatment direction of the programMaintains the highest standard of professional conduct in relation to information that is confidential in nature. Shares information only when the recipient’s right to access is clearly established and the sharing of such information is clearly in the best interest of the person servedAdheres to and participates in Company’s mandatory Health Insurance Portability Accountability Act privacy program/practices and Business Ethics and Compliance programs/practicesAttends, participates in and/or conducts internal staff development programs, obtain continuing education as required by Company policy and regulations. Maintains and enhances clinical practice skillsPerforms other duties assigned Qualifications Bachelor’s degree in Rehabilitation/Counseling, Education, or Human Service/Allied Health field or the equivalent and Minimum of two years case management and/or related administration experience requiredExperience in a residential or rehabilitation setting with neurologically impaired clients preferredExperience managing program and direct reports preferredDemonstrates effective and professional interpersonal, verbal, and written communication skillsStrong computer software (Microsoft Office) and data entry skills Able to work independently and part of a teamOrganized and able to function under minimal supervisionHas reliable transportation and is able to travel to/from multiple assignments/locations on a daily basis About our Line of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
