About Duke HomeCare & Hospice: Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, and serves as the home for the Duke Caregiver Support Program. Team members work closely with a patient’s physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham. Job Summary As a Home Health Licensed Practical Nurse with Duke University Health System, you will prioritize providing service while caring for the whole person in a patient‑centered atmosphere. The Home Health LPN assumes responsibility for direct nursing care of assigned patients under the supervision of a registered nurse or physician. You will provide nursing services to patients and families in accordance with the scope of the LPN as defined by the North Carolina Board of Nursing. Responsibilities Provide direct nursing care to patients and families in a home health setting. Perform duties outlined by the scope of licensed practical nurse, including delegating to nursing support staff. Collaborate with the interdisciplinary team to develop and implement patient care plans. Teach patients and families about care, interventions, and health management. Document interventions and maintain accurate patient records. Use computer systems and learn new software programs as required. Remain focused, organized, and consistent with BLS or other life‑support requirements. Function within an 8‑hour shift model, Monday to Friday, with rotating Saturday shift approximately every 7‑8 weeks. Qualifications Graduation from an accredited Practical Nurse program. Current North Carolina LPN licensure or Multi‑State Compact License (must obtain permanent NC license within 60 days if from a compact state). Active and current driver’s license and automobile insurance. BLS certification (expired renewal provided during orientation). Highly preferred: 1+ year of home‑health experience, or high‑acuity experience including wound‑vac. Knowledge of the care and treatment of patients and special procedures applicable to practical nursing. Ability to function independently, communicate effectively with patients, families, and the healthcare team, and use fine‑motor skills. Benefits Quality of Life: Living in the Triangle.
