RN - Case Management

Acute Care Case Manager

This is a 13-week contract position.

Day Shift: 5x8 hours (08:00 - 16:30)

The Acute Care Case Manager will be responsible for managing patient care through effective case management and discharge planning. The ideal candidate will have a strong background in acute care settings and demonstrate proficiency in coordinating care and facilitating patient transitions.

Oversee a case load of patients, ensuring quality care and adherence to established care plans.

Coordinate discharge planning and facilitate smooth transitions for patients.

Conduct assessments and develop treatment plans integrating evidence-based guidelines.

Collaborate with interdisciplinary teams including physical therapy, social services, and pharmacy.

Communicate with families and advocate for patients' needs and preferences.

2 years of experience in acute care case management or discharge planning.

Active California state license required; pending licenses accepted upon start date.

Experience with Epic charting system is required.

Knowledge of Medicare guidelines and related insurance procedures.

Experience in coordinating care with interdisciplinary teams for diverse patient populations, including pediatrics and geriatrics.

Every other weekend availability is required. Must also be available to work on holidays.
