Community Case Manager

Job Details Job Location: Newton, KS 67114 POSITION RESPONSIBILITIES: CLINICAL WORK * ADMINISTRATIVE RESPONSIBILITIES * BILLABLE NORM * ABILITY TO TRANSPORT PATIENTS * COMMUNITY EDUCATION * PROFESSIONAL DEVELOPMENT * PERFORMANCE IMPROVEMENT EMPLOYEE CONTRIBUTES TO POSITIVE WORK ENVIRONMENT WITH COWORKERS AND CUSTOMERS CPA 1: CLINICAL WORK * Provide competent community based services in the community, school, and patient’s home, as assigned by supervisor and as specified in the patient’s treatment plan provided from a qualified mental health provider. Provide community based services based on the philosophies and principles of emphasizing patient’s strengths. Assist staff/clinicians in identifying, providing outreach to, and establishing rapport with new community based services patients. Communicate regularly with referral sources at admission, as needed during treatment, and at termination, if appropriate. Provide case consultation services and attend treatment meetings with internal staff and external agencies and organizations, as needed. Maintain clinical outcomes within Prairie View standards. Provide competent services to patients in Prairie View’s Summer Intensive Rehabilitation Program (SIRP). Provide community based services to the patient that comply with ethical standards and procedures. Engage in the following patient centered community based activities as needed by each patient/family: Highlight patient/family’s strengths and successes to help improve their self esteem. Employ problem solving with patient/family to help them resolve issues and problems. Provide effective crisis intervention services to the patient and their family. Assess strengths and set goals with the patient/family; review regularly. Help the patient/family become linked with necessary community services/resources. Teach life and parenting skills to patient/family. Encourage patient/family to become more actively involved in the community. Provide support and advocacy with various systems on behalf of patient/family to facilitate obtaining needed resources and/or protection of rights. Be flexible in providing services and needed support to our patients and their families during times that would best fit the patients’ needs. Demonstrate competency in working with child/adolescent patients and their caregivers. Demonstrate an understanding of differences between typical and atypical child/adolescent behavior. Demonstrate an understanding of healthy child/adolescent environmental structures and routine, as evidenced by: Effective provision of individual and group interventions. Offering effective input into team building, and/or treatment planning. Demonstrate the ability to establish and maintain appropriate limits and boundaries when working with children/adolescents and their caretakers, both individually and in group work. Demonstrate knowledge of risk factors for individual children/adolescents in regards to personal safety, safety in the community, and worker safety; demonstrate knowledge of appropriate interventions to keep the patient and others safe. If participating in psychosocial group work assignments, demonstrate knowledge of group management strategies, and best practice psychosocial group skill curriculum development. CPA 2: ADMINISTRATIVE RESPONSIBILITIES * Attend administrative and departmental meetings as scheduled. Maintain necessary personal records to document time and energy expended on behalf of the patient/family. Maintain adequate and timely documentation in the clinical record of treatment interventions and other activities related to patient care. Work cooperatively with other professional staff within the office, in service of the patients. Participate regularly in clinical and administrative supervision with supervisor and peers. Maintain responsibility for organizing a schedule and establishing priorities, according to existing patient and agency needs. Work cooperatively with Utilization Review staff and Treatment Team to assure authorization is in place to provide services, and specific services are identified on the active Treatment Plan. Insure that medical records are completed, as standards require: Progress notes are completed within 48 hours. CSR’S monthly information is completed by the 5th of every month CPA 3: BILLABLE NORM * Maintain monthly expectations. (IDENTIFY WHAT THEIR ENCOUNTER EXPECTATIONS ARE) CPA 4: ABILITY TO TRANSPORT PATIENTS * Maintain a valid Kansas Driver’s License. Provide proof of current automobile insurance on your personal vehicle. Pass Driver’s License check. CPA 5: COMMUNITY EDUCATION * Provide education as necessary to those in the broader community who come in contact with the children’s community based services program. CPA 6: PROFESSIONAL DEVELOPMENT * Complete required CCM state training within 6 months of employment. Develop and discuss annual plan for continuing education with supervisor. Participate in professional workshops, continuing education and conferences to maintain and improve clinical skills and expand professional knowledge. Use knowledge and creativity to expand and develop new ideas in collaboration with colleagues and supervisors. Participate in continuing education activities and required in services (such as Infection Control, etc.) as requested, to enhance and improve job performance. Utilize Relias Learning site to enhance knowledge in the areas of mental health and treatment modalities. CPA 7: QUALITY ASSURANCE & PERFORMANCE IMPROVEMENT (QAPI) Participate in organizational Prairie View, Inc. activities. Be familiar with Prairie View, Inc. Initiatives and goals in work area. Annually review the Quality Assurance & Performance Improvement (QAPI) Plan. CPA 8: EMPLOYEE CONTRIBUTES TO POSITIVE WORK ENVIRONMENT WITH COWORKERS AND CUSTOMERS Treats others with respect and dignity. Maintains a flexible response to organizational and customer needs. POSITION DESCRIPTION This position description is a temporary management guide tool and is subject to change. QualificationsPOSITION QUALIFICATIONS: Minimum Education: Bachelor’s degree in related field of behavioral health sciences or equivalent Minimum Experience: Demonstrated skills in the provision of case management Must be at least 20 years of age Must have good listening skills, verbal skills, and written communication skills Must be able to observe patient demeanor and nonverbal communication for assessment purposes Must be able and willing to have some flexibility in scheduling, to meet patient and organization needs Must maintain Kansas State licensure and appropriate CEU’s where applicable Must be able to communicate effectively with referral sources and external care providers Must have ability to work effectively with children, adolescents, and their families Must be able to understand the special needs and behaviors of and be able to care for children and adolescents ages 2 to 18 (up to 22 years old if on the Waiver) Must have a valid Kansas driver’s license and be insurable through Prairie View insurance company. Must be able to pass physical restraining program Must be able to bend, squat, and kneel to do physical activities with children Must be proficient at basic keyboarding/word processing skills Must be able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check #J-18808-Ljbffr
