You'll play a key role in improving patient outcomes, preventing re-hospitalizations, and helping patients regain independence safely at home.Key Responsibilities:Deliver skilled nursing care according to physician orders and care plansMonitor patient conditions and communicate changes appropriatelyAdminister medications and treatments as prescribedEducate patients and families on disease processes and preventative careComplete accurate and timely clinical documentationFollow agency policies, procedures, and Medicare/Federal guidelinesParticipate in trainings and team meetingsRequirements:Current Licensed Practical Nurse (LPN) license in the state of employmentMinimum of one year of LPN experience in an acute care settingHome health experience preferredValid driver's license and active auto insuranceAbility to meet physical job requirementsWe are proud to be part of the Alternate Solutions Health Network family. #INDGHC8We'll help you put your passion for patient care to work.
