Key Responsibilities
- Provide direct physical therapy services to patients in an outpatient therapy setting
- Evaluate, assess, and develop individualized treatment plans based on patient needs
- Implement therapeutic interventions and monitor patient progress toward goals
- Complete required clinical documentation accurately and in a timely manner
- Collaborate with the care team to ensure coordinated and effective patient outcomes
Required Qualifications
- Active Physical Therapist license in the state of Indiana (permanent license required)
- Prior experience in an outpatient or relevant physical therapy setting
- Ability to meet facility-specific credentialing and documentation requirements prior to assignment start
- Candidates who have worked in a permanent or PRN capacity at this facility within the last year are not eligible for this assignment
Preferred Experience / Skills
- Experience working in a high-volume outpatient therapy environment
- Familiarity with travel or contract therapy assignments
