The role involves a structured schedule from Monday to Friday, totaling 40 hours a week, with occasional Saturday rotations to ensure comprehensive patient care.

Key Responsibilities

• Conduct comprehensive assessments of patients, identifying both normal and abnormal findings to develop effective treatment regimes.
• Manage the physical and psychological aspects of patient care, utilizing evidence-based practices.
• Develop and implement individualized treatment plans tailored to each patient’s unique needs, ensuring regular reassessment and adaptation as necessary.
• Monitor and evaluate patient responses to medications and treatments, adjusting plans to optimize health outcomes.
• Prescribe medications and immunizations according to established guidelines, while managing potential drug interactions and side effects.
• Initiate and maintain an up-to-date Problem/Medication list for each patient encounter for precise tracking of health changes and treatment progress.
• Ensure complete and accurate documentation at every encounter to comply with regulatory standards and policies.
• Collaborate with physicians and a multidisciplinary team to develop patient care strategies, ensuring seamless communication and patient transitions across the continuum of care.

Patient Population / Client Focus

The Cardiology Nurse Practitioner will primarily work with patients suffering from various cardiovascular disorders, including hypertension, arrhythmias, heart failure, and other related conditions. The role requires a sensitive understanding of patient concerns and the ability to address the complex emotional and physical aspects of cardiovascular health. Our diverse patient population necessitates a practitioner who can engage with individuals from various backgrounds, adapting care strategies to meet their distinct needs.

Qualifications

• A Master’s degree in Nursing from an accredited Nurse Practitioner program.
• A valid Registered Nurse (RN) license issued by the State of New York.
• Basic Life Support (BLS) certification.
• Strong interpersonal and communication skills to foster effective relationships with patients and the healthcare team.

Preferred Experience

• A minimum of 2-3 years of clinical experience in cardiology is highly preferred to ensure familiarity with common procedures and patient types within this specialty.

Compensation and Benefits
