Lifted Hospice is seeking an On-Call Registered Nurse (RN) Case Manager to deliver expert hospice care in patients' homes or care settings. You will lead the bedside experience, manage symptoms, and guide families through care plans in collaboration with the interdisciplinary team. The role requires an active nursing license, strong interpersonal skills, and the ability to educate and support families during end-of-life care.
