East Haven Nursing and Rehabilitation Center - About Easthaven Nursing & Rehabilitation Center Easthaven Nursing & Rehabilitation Center is dedicated to providing exceptional skilled nursing, rehabilitation, and long-term care services in a compassionate and resident-centered environment. We are seeking a Registered Dietitian to join our interdisciplinary team and help ensure the nutritional health and well-being of our residents. Position Summary The Registered Dietitian is responsible for assessing, planning, implementing, and monitoring nutrition care for residents. The ideal candidate will work closely with physicians, nursing staff, speech therapists, and food service personnel to develop and maintain individualized nutrition plans that support residents' medical needs, recovery, and quality of life. Responsibilities Complete comprehensive nutrition assessments for new admissions and ongoing resident evaluations. Develop, implement, and monitor individualized nutrition care plans. Conduct regular nutritional screenings and identify residents at nutritional risk. Monitor weight trends, laboratory values, wound healing, hydration status, and dietary intake. Collaborate with the interdisciplinary care team to ensure optimal resident outcomes. Provide recommendations for therapeutic diets, supplements, and nutrition interventions. Participate in resident care conferences and quality improvement initiatives. Ensure compliance with federal, state, and local regulations, including CMS and Department of Health requirements. Educate residents, families, and staff on nutrition-related topics as appropriate. Maintain accurate and timely documentation within the electronic medical record system. Work closely with the Food Service Director to ensure menus meet nutritional standards and resident needs. Qualifications Bachelor's Degree in Dietetics, Nutrition, or related field. Registered Dietitian (RD/RDN) credential required. Current New York State certification/licensure as required. Experience in long-term care, skilled nursing, rehabilitation, or clinical nutrition preferred. Strong knowledge of medical nutrition therapy, geriatric nutrition, and regulatory requirements. Excellent communication, organizational, and interpersonal skills. Ability to work independently and collaboratively within a multidisciplinary team.
