Fee For Service Registered Nurse (RN)

Join our compassionate homecare team and make a meaningful difference in the lives of patients throughout Brooklyn and Queens. We are seeking dedicated Registered Nurses (RNs) who are passionate about providing exceptional home health care to Adult Geriatric and Pediatric populations.

Position Type: Per Diem (Fee for Service)

Minimum Commitment: 25 patient visits per week

Compensation:

• $85 per routine visit
• $95 per recertification/discharge
• $120 per admission

Responsibilities:

• Provide skilled nursing care to patients in their homes across Adult Geriatric, Women's Health, and Pediatric populations.
• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications, treatments, and other physician-ordered interventions.
• Complete admissions, recertifications, and discharge visits.
• Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management.
• Monitor patient progress and communicate changes in condition to the appropriate healthcare providers.
• Maintain accurate, timely documentation in accordance with agency, state, and federal regulations.
• Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to ensure quality patient care.

Qualifications:

• Active Registered Nurse (RN) license in New York.
• Experience in Adult Geriatric, Women's Health, and/or Pediatrics preferred.
• Home health or community nursing experience is a plus.
• Strong clinical judgment, assessment, and communication skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation to travel throughout Brooklyn and Queens .
