Social Worker – Full-Time
Location: North Dauphin Nursing and Rehab – Millersburg, PA

North Dauphin Nursing and Rehab is seeking a dedicated and compassionate Social Worker to join our skilled nursing team in Millersburg, PA. In this role, you will serve as a vital advocate for residents, supporting their emotional well-being, assisting families, and facilitating smooth transitions throughout their stay.

Key Responsibilities

• Complete comprehensive social service assessments and develop individualized care plans that address residents’ psychosocial needs with realistic, measurable goals.

• Actively participate in pre-admission and admission processes to ensure appropriate placement and smooth transitions.

• Provide ongoing emotional support and crisis intervention to residents and families.

• Document behavioral, emotional, and psychosocial updates in compliance with facility policies and state and federal regulations.

• Facilitate individual and group support sessions as appropriate.

• Develop and coordinate volunteer programs tailored to resident interests and needs.

• Orient new residents and families to the facility, services, and available resources.

• Collaborate with the interdisciplinary team to ensure holistic, resident-centered care.

Qualifications

• Bachelor’s degree in Social Work or a related field preferred.

• Knowledge of long-term care social services practices, regulations, and compliance requirements.

• Strong interpersonal, communication, and organizational skills.

• Ability to work independently while contributing effectively to a multidisciplinary team.

• Demonstrated empathy, professionalism, and problem-solving ability.
