About the Role:

The Licensed Practical Nurse (LPN) Night position at Van Buren House is a critical role focused on providing compassionate and skilled nursing care to residents during nighttime hours. The primary goal is to ensure the health, safety, and comfort of all residents by monitoring their conditions, administering medications, and responding promptly to any medical needs or emergencies. This role requires close collaboration with the healthcare team to maintain accurate documentation and support individualized care plans. The LPN will also play a vital role in observing changes in residents' health status and communicating these observations to the appropriate medical personnel. Ultimately, this position contributes to maintaining a safe and supportive environment that promotes the well-being and dignity of each resident throughout the night shift.

Minimum Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of employment.
• High school diploma or equivalent.
• Basic Life Support (BLS) certification.
• Previous experience in a long-term care or skilled nursing facility preferred but not required.
• Ability to work overnight shifts and adapt to a nighttime work environment.

Preferred Qualifications:

• Experience working in geriatric or long-term care settings.
• Additional certifications such as IV therapy or wound care.
• Strong knowledge of electronic health record (EHR) systems.
• Demonstrated ability to handle emergency situations calmly and effectively.
• Excellent communication and interpersonal skills to interact with residents, families, and healthcare team members.

Responsibilities:

• Administer prescribed medications and treatments accurately and timely during the night shift.
• Monitor residents’ vital signs and overall health status, promptly identifying and reporting any changes or concerns.
• Provide direct nursing care including wound care, catheter care, and assistance with activities of daily living as needed.
• Document all nursing interventions, observations, and resident responses in compliance with facility policies and regulatory requirements.
• Collaborate with nursing staff, physicians, and other healthcare professionals to implement and update individualized care plans.
• Respond quickly and effectively to medical emergencies or resident needs during the night hours.
• Maintain a safe, clean, and comfortable environment for residents, ensuring adherence to infection control and safety protocols.

Skills:

The required skills such as medication administration, patient monitoring, and documentation are essential for ensuring residents receive safe and effective care throughout the night. The LPN uses clinical judgment daily to assess resident conditions and make timely decisions that impact health outcomes. Preferred skills like proficiency with electronic health records enhance efficiency and accuracy in maintaining resident information. Strong communication skills facilitate clear and compassionate interactions with residents and collaboration with the healthcare team. Additionally, emergency response skills are critical for managing unexpected situations calmly and effectively, ensuring resident safety at all times.
