Key Responsibilities\n

• Collaborate closely with the healthcare team to devise and implement individualized treatment plans, including comprehensive discharge plans based on detailed psychosocial assessments.

• Conduct thorough patient assessments to identify needs, develop actionable treatment modalities, and monitor patient progress, ensuring clinical and progress notes are maintained according to established standards.

• Provide immediate intervention in crisis situations, facilitating assessment and management of emotional and behavioral challenges that patients may encounter.

• Engage with family members and support systems to coordinate patient treatment, enhance continuity of care, and assist with effective discharge planning.
