We are currently hiring a Community Transition Liaison! CTL acts   as onsite point of contact for residents, families, nursing facility (SNF) staff, and all other parties involved with resident’s care for transitions from the NF to the community. Visits with residents to increase awareness of service and introduce transitions to the community as a potential option.

The range for the hourly rate is $28.40 – 31.55 per hour.

Monday- Friday, 9a-5p: 34 hours/ week plus unpaid one-hour lunch daily. One day will be one hour shorter than the other 4 days.

What do you need to be considered for this position?

• Ability to collaborate, take initiative, provide/ receive feedback.
• Ability to work independently and professionally.
• Excellent computer skills required, including MS Office 365.
• Excellent in-person and written communication skills.
• Experience with handling sensitive information, including maintaining confidentiality.
