Care Coordinator - CKD Navigation
Virtual Vocations Inc
To support patients with chronic kidney disease, the remote Care Coordinator - CKD Navigation will manage patient engagement, care coordination, and resource navigation, ensuring comprehensive support and improved health outcomes through collaboration with an interdisciplinary team. Key responsibilities Perform telephonic outreach to enroll and engage patients in clinical programs, serving as the primary point of contact Develop and implement care plans in collaboration with the Nurse Care Manager, addressing the unique needs of patients Conduct assessments to identify social determinants of health and connect patients to necessary resources for improved care Required qualifications 3 years of experience in healthcare coordination or community health work Experience with a team-based approach to care Strong knowledge of community resources and services Intermediate skills with MS Office Suite, including Outlook and Teams Ability to work effectively in a remote environment with required internet speeds
