Womens Health Care Coordinator

Summary/ObjectiveThe Women’s Health Coordinator (WHC) is responsible for coordinating maternal and women’s health care to achieve the best possible health results for mothers and newborns. The WHC will help coordinate maternity services, prenatal education programs, gynecological care coordination, addressing social factors, connecting to resources, and hospital coordination. The WHC works closely with the Pediatric Care Coordination team to ensure proper transition of care.Essential FunctionsEnsures pregnant women are obtaining prenatal care, and consistently making prenatal appointmentsPerforms Non-Stress Test per doctor’s orders, monitoring fetal wellbeingReinforces visit summary instructions from various service providersAssists with referrals to WIC, Maternal/Fetal Medicine, requests for durable medical equipment, etc.Assists in obtaining pre-birth selection forms and entering information into the Indiana State databaseObtains initial medical and obstetric history, provides pregnancy related education for prenatal patientsAssists patients in making informed decisions regarding family planning options by providing effective education and resourcesProvides education and resources regarding women’s health conditionsAssists clinical staff by performing free pregnancy tests, covering lunch/breaks, etc.Keeping open and ongoing communication between patients, their families, and healthcare providersAssists at developing educational classes and programs for at risk pregnant women at NorthShore and our servicing communitiesTrain and educate patients and their families on care coordination, creating SMART goals, available organizational and community services, and self-management techniquesCompletes proper documentation in patient medical records, as well as productivity managing softwareMaintain Indiana Child Passenger Safety Technician certification by completing seat safety checks, education and installationsAttends staff meeting, department training/meetings, etc.Promote NorthShore services to patients and families and assist with transition of care to the Pediatric Care Coordination teamCoordinate access to prenatal care and linkage to services that include medical home health, healthy eating, centering classes, breast feeding classes and other services based on client needsOther related duties assigned by the Care Coordination ManagerThese essential functions are a summary of the primary duties and responsibilities of the position and are not intended to be a comprehensive listing of all duties and responsibilities.
