RN Case Manager – Job Overview Journey Hospice offers a rewarding and fulfilling full‑time RN Case Manager position covering a caseload in Downingtown, PA. Responsibilities The Hospice RN will oversee a primary caseload of Hospice patients and provide skilled professional nursing care to patients, families, and caregivers as prescribed by the physician and in compliance with Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures. Responsibilities include providing initial assessment and comprehensive nursing assessment, identifying problems, reassessing nursing needs, and collaborating to develop the Hospice Interdisciplinary Team (IDT) plan of care. The RN implements the plan of care, reevaluates and revises it as needed, and teaches, supervises, and counsels patients, family members, and staff regarding nursing care and related issues at home. The RN works closely with other IDT members to ensure all patient and family care needs are met and provides supervision to Hospice Aides/Homemakers. Education / Training / Certifications Graduate of an accredited registered nursing program. Bachelor’s degree in Nursing preferred. Certified Hospice and Palliative Nurse (CHPN) certification preferred. PA RN Licensure by the PA State Board of Nursing in good standing. Valid driver’s license and reliable transportation insured under PA state requirements. Skills and Knowledge Maintain a professional attitude and demeanor; communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, and others. Demonstrate optimism, enthusiasm, and willingness to work constructively with other healthcare team members and reflect concern for the well‑being of patients and employees. Interact with all staff positively, supporting the mission and vision of Journey Hospice. Experience Minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical, or critical care experience. Experience with pain and symptom management interventions preferred. Ability to work within an interdisciplinary setting.
