We offer a competitive salary and a comprehensive benefits package for eligible full-time staff: Starting salary around $47,500/year plus service productivity bonuses Health, dental, and vision insurance (after obtaining full-time status) 401(k) with company eligibility after one year of full-time employment{{newline}} Paid maternity leave Hands‑on professional experience and career development Bachelor’s Degree in a human services field Valid driver’s license and reliable transportation Ability to pass NCIC/CIB and APS/CPS background checks Three professional references Strong written and verbal communication skills Proficiency with Outlook, Word, Excel, and PDF platforms Professionalism and strong customer service skills Ability to work independently with minimal supervision Ability to work evenings and weekends as needed Experience with virtual platforms Preferred 2–4 years of case management and/or behavioral health experience 1–2 years of experience leading teams Experience providing direct services to children and families Key Responsibilities Provide wraparound services to a caseload of up to 15 families Coordinate supports, resources, and services using a strengths-based approach Facilitate team meetings with youth, families, and multidisciplinary partners Collaborate with MCO care managers and community resources Conduct and administer the Child and Adolescent Needs and Strengths (CANS) assessment Monitor service implementation and ensure families receive identified supports Lead planning from engagement through transition and discharge Conduct home visits according to each family’s individualized plan Identify service gaps and collaborate with community providers to address needs Maintain accurate, timely, and compliant documentation Participate in scheduled supervision and coaching Maintain client records in alignment with agency standards and regulatory guidelines Physical Requirements Ability to walk up and down stairs Ability to lift 5–50 pounds Ability to kneel, bend, squat, and stand Ability to operate a motor vehicle and follow all state traffic laws Home Base Inc. is an Equal Opportunity Employer (EOE). #J-18808-Ljbffr Home Base Inc.
