Registered Nurse (RN) Case Manager - Home Health

Job Summary Connected Home Health is seeking experienced and caring Home Health Registered Nurse (RN) Case Managers to join our team. The role focuses on providing flexible, patient‑centered care and supporting a team of skilled clinicians in the greater Portland, OR area. Essential Job Functions / Responsibilities Provide direct patient care as defined in Oregon’s Nurse Practice Act. Implement plans of care and provide accurate, timely documentation consistent with those plans. Assess and offer patient and family/caregiver education related to diagnosis and treatment. Coordinate home health services, appropriately reporting identified needs to the registered nurse. Position Qualifications Graduate of an accredited practical nurse or vocational nursing program. Minimum one year of nursing experience preferred. Currently licensed registered nurse in the state of Oregon. Demonstrated compliance with professional standards and practice. Effective verbal and written communication with strong organization skills. Valid driver’s license and reliable, insured automobile in good working order. Current CPR/BLS certification from AHA.
